Welcome!
In the section on “General Information - Matrix” you were lead through a series of questions that helped you to determine which service you needed to apply for through FCCPT. Now that you are ready to apply for a service, it is helpful to know what you need to get started with the application process.
Online applications
- All applications are completed online. FCCPT no longer accepts paper applications.
- You will find the “Apply Now” button on the description pages for each Primary Service.
- View a Sample Application.
Making a payment
- Full payment using a credit card, is due at time of application
- FCCPT accepts Mastercard and Visa only.
- Please have your card handy at the time of application.
Withdrawing an application
- Applicants may withdraw an application within five(5) business days.
- To withdraw submit a letter of explanation by mail or faxed to 703-684-8715.
- Letter to include applicant’s full name, file number, and signature.
- There is a $100 service fee for withdrawing an application.
- There will be no refund for a withdrawal after the fifth day.
- If an applicant subsequently reopens an application, the full fee will be required at that time.
Changing a Service
- Applicants may request a change in the service by sending a letter by mail or FAX 703-684-8715.
- Letter to include applicant’s full name, file number, and signature.
- There is a $100 service fee for changing services,
- Additional fee if the new service is a higher cost.
Existing files with FCCPT
If you have had a previous service with FCCPT you should use the existing file number.. If you cannot remember the number, or are unable to access your previous file for any reason, do not open a new file. Contact FCCPT by e-mailing help@fccpt.org.
Opening a new file can cause a potential 60 day delay in processing your service request.
