If I have had a previous service with FCCPT, do I have to have the documents sent again?
If an applicant has had any of FCCPT’s services completed in the past and has documents on file in our office, most will not be acceptable for the New York Verification Process. Only the PT diploma can be used for other services applied through FCCPT. There is a specific collection process for the documents involved in this service. Please read these instructions carefully to understand the stipulations placed on this process.

If I have had a previous NY verification and want a new service do I have to request the documents again?
If the applicant wants to get a physical therapy license to work in another jurisdiction, the applicant can apply with FCCPT for either the Comprehensive Credential Evaluation (Type I Service), or the Educational Credentials Review Service. All documents collected for the New York Verification Process are kept on file at FCCPT and will be used for additional services.

Once I pay for the NY verification, do I still have to apply to the state for licensure?
Yes, the licensure application is a totally separate process, see the NY contact information and read carefully the instructions for applying at www.op.nysed.gov/prof/pt.

What happens if my school and/or licensing agency does not respond?
Applicants who have ordered a New York Verification Process are not eligible for re-activation of the service.

For the New York Verification process, FCCPT will make four requests to an educational institution or licensing/registration authority, approximately every 60 days. If, at the end of 240 days, there is no response from the institution or licensing/registration authority and FCCPT has completed its four requests, the service will be deemed complete, and the file will be forwarded to NYSED as unable to authenticate.

If an applicant wants to re-open the file and continue to have FCCPT send the request letters to the educational institution or licensing/registration authority, a new service of the New York Verification Process will need to be ordered. Applicants will not be able to order this second service online. Instead, an applicant must fax or mail a letter to request to add this second New York Verification Process to their online file. The letter must include the applicant’s full name, file number, and signature. The fax number for FCCPT is 703-684-8715.

The applicant will need to submit new request forms for all institutions that did not provide the documents in the initial process to FCCPT. It is recommended that prior to opening the service again the applicant follow up with the institution to find out why they had not responded before to avoid the same issues again.
