What the applicant will do

What the applicant will NOT do

  • Applicants will NOT send original diplomas or certificates.
    • All documents sent to the FCCPT become the property of the FCCPT and will not be returned.
  • Applicant will NOT send any copies of transcripts, mark sheets, syllabi, or license verifications to FCCPT.

  • Applicants will NOT collect transcripts or verifications in sealed envelopes and forward these to the FCCPT.

  • Any documents that are not required will not be maintained in the FCCPT files.

What the Academic institution will do

Actions the academic institution does NOT take

  • The academic institution will fill out the NYSED Academic Verification Form that FCCPT sends and submit it with the official transcript and official course descriptions and/or syllabi to FCCPT.
  • The academic institution will not send any documents to the applicant to forward on to FCCPT.

What the licensing authority will do

Actions the licensing authority does NOT take

  • The licensing/registration authority will fill out the NYSED Licensure Verification Form that FCCPT sends and submit it to FCCPT.
  • The licensing authority will not send any documents to the applicant to forward on to FCCPT.

What Will FCCPT do

Actions FCCPT does not take

  • FCCPT will supply the applicant with a file number once an online payment and application have been completed.

  • FCCPT will send four requests to the school and licensing authorities.

  • FCCPT will send requests every 60 days.

  • FCCPT will verify and authenticate the documents according to the NYSED regulations and send to NYSED.

  • If the institutions do not respond after four attempts then the FCCPT will send what documents have been verified to NYSED, and applicant will follow up with the state at that time.

  • FCCPT will send a duplicate Coursework Tool report to NYSED with verification documents if requested by individuals who have had a previous CE or Type I review.
  • FCCPT will not do an educational credentials review. Once documents have been sent to the NYSED the applicant should follow up with the NY board. The FCCPT does not have any additional contact with the files.

  • FCCPT does not issue the license, the jurisdiction board does this.

  • FCCPT will not re-activate a file after four attempted requests to the agencies. An applicant will need to apply for a new service if they wish us to make more attempts.

  • The FCCPT will NOT give out specific information to any third party without a notarized release of information from the applicant.
