Submitting Documents to FCCPT

All documents should be submitted to the following address.

• Applicants are to submit their notarized attestation page and a notarized copy of the PT diploma
• Academic institutions and licensing authorities are to submit official documents directly to this address.
  • When a courier service, such as FedEx, DHL, etc., is used, the representative from the institution must place the sealed documents in the courier package.
  • Do NOT arrange for third parties to pick up documents in an effort to expedite their delivery. FCCPT cannot verify that these are official documents if they are not sent directly from the institution issuing the documents
  • If there is any question of document mishandling, FCCPT will verify the documents authenticity. This will delay the processing of the final report.
