What the applicant will do

What the applicant will NOT do

  • Applicant will provide all required documents within 12 months of the date of application.

  • Applicant will log into account and “view progress” to determine what checklist items are to be requested and sent to the FCCPT.

  • Applicant will send academic and licensing request forms to the appropriate institutions
  • Applicant MUST hold a current active and unencumbered US license. ALL licenses must be reported and verified.
  • Applicant will contact the FSBPT (www.fsbpt.org) for the score transfer for the NPTE.
  • Applicant will submit a notarized attestation page with a recent photo to FCCPT.
  • Applicant will send a notarized copy of the PT diploma to FCCPT.

  • Applicant will contact ETS (www.ets.org) and ask them to send TOEFL scores to FCCPT. The FCCPT Institution code is 8164.

  • The applicant will submit a notarized release of information if they wish a third party to contact the FCCPT in their place.
  • Applicant may not request a change of service from a type 2 to another service, after December 31, 2009. Prior to that time an applicant may make a change by requesting the change in writing with a signature, paying the difference in fees and paying a $100 service charge.

  • Applicants will NOT send original diplomas or certificates.
    • All documents sent to the FCCPT become the property of the FCCPT and will not be returned.
  • Applicant will NOT send any copies of transcripts, mark sheets, syllabi, or license verifications to FCCPT.

  • Applicants will NOT collect transcripts or verifications in sealed envelopes and forward these to the FCCPT.

  • Any documents that are not required will not be maintained in the FCCPT files.

What the Academic institution will do

  • The academic institution will fill out the academic verification form and submit it with the full official transcript.
  • The academic institution will not send any documents to the applicant to forward on to FCCPT.

What the licensing authority will do

Actions the licensing authority does NOT take

  • The U.S. state licensing authority and ALL other licensing authorities will fill out the Verification of Licensure Form and submit it to FCCPT.
  • The licensing authority will not send any documents to the applicant to forward on to FCCPT.

Actions the examining institution take

Actions the examining institution does NOT take

  • ETS will forward the applicant’s TOEFL, TWE, TSE, and/or IBT test scores to FCCPT for the English proficiency examinations.
  • The examining institutions will not send any documents to the applicant to forward on to FCCPT.

What FCCPT will do

Actions FCCPT does NOT take

  • FCCPT will supply the applicant with a file number once an online payment and application have been submitted.

  • FCCPT will verify all documents as authentic.

  • FCCPT will process the Type 2 certificate once all documents are in our offices and verified within 15 working days.

  • FCCPT will mail the certificate via US mail services unless the applicant has provided a Federal Express Account number with instructions for sending the document.

  • For the Type II Process, FCCPT will not be contacting any academic, licensing, or examining institutions on behalf of the applicant. However, FCCPT may contact academic institutions, licensing/registration agencies if further verification is needed.

  • The FCCPT will NOT give out specific information to any third party without a notarized release of information from the applicant.
