NY Credentials Verification

For licensure in New York, FCCPT offers the New York Credentials Verification Service.

To obtain a Healthcare Worker Certificate, you will also need the Comprehensive Credentials Review (Type 1 Review). You should apply for BOTH services; however, to avoid delays, please follow the document collection process for the New York Credentials Verification. Documents received will be accepted for both services if received from your Issuing Institution(s) at FCCPT request, as New York requires.

FCCPT will only send a completed evaluation report to New York as an advisory report, and this can only be requested if you have completed the New York Credentials Verification process. If you have already completed the New York Credentials Verification Service AND also have a completed evaluation report from FCCPT, then a Duplicate Report may be right for you.
