Overview of the steps in the PTA-EER

The Physical Therapist Assistant Educational Equivalency Review Service:

This review is developed to meet the requirements of the Medicare/Medicaid requirments. It may be used for licensure or registration in some states.

What the applicant will do

What the applicant will NOT do

  • Applicant will check with jurisdiction prior to application to determine if the jurisdiction accepts the reviews.

  • Applicant will apply and pay online for the PTA-EER service.

  • Applicant will log into account and “view progress” to determine what checklist items are to be requested and sent to the FCCPT.

  • Applicant will keep profile and e-mail address current.

  • Applicant will send academic and licensing request forms to the appropriate Institutions.
  • Applicant will submit a notarized attestation page for the PTA-EER service with photo to FCCPT.
  • Applicant will send a notarized copy of the PTA diploma to FCCPT.

  • The applicant will submit a notarized release of information if they wish a third party to contact the FCCPT in their place.
  • Applicants will NOT send original diplomas or certificates.
    • All documents sent to the FCCPT become the property of the FCCPT and will not be returned.
  • Applicant will NOT send any copies of transcripts, mark sheets, syllabi, or license verifications to FCCPT.

  • Applicants will NOT collect transcripts or verifications in sealed envelopes and forward these to the FCCPT.

  • Any documents that are not required will not be maintained in the FCCPT files.

What the academic institution will do

Actions the academic institution does NOT take

  • The academic institution will fill out the academic information release form and submit it with the full transcript and official course descriptions and/or syllabi to FCCPT.
  • The academic institution will not send any documents to the applicant to forward on to FCCPT.

What FCCPT will do

Actions FCCPT does NOT take

  • FCCPT will supply the applicant with a file number once an online payment and application have been completed.

  • FCCPT will collect and verify ALL documents as they arrive.

  • FCCPT will complete the PTA-EER within 12 weeks and provide a report to the applicant and one jurisdiction.

  • FCCPT will send all reports electronically to the e-mail address on file.
  • For the PTA-EER service, FCCPT will not be contacting any academic, licensing, or examining institutions on behalf of the applicant, unless we need further verification.

  • The FCCPT will NOT give out specific information to any third party without a notarized release of information from the applicant.
