What the applicant will do

What the applicant will NOT do

  • Applicant will apply and pay online for the Type I Service. (see fee schedules).

  • Applicant will log into account and “view progress” to determine what checklist items are to be requested and sent to the FCCPT.

  • Applicant will send academic and licensing request forms to ALL the appropriate institutions. (US and Foreign)
  • Applicant will submit a signed and notarized Type I attestation page with one (1) recent passport-type photograph.
  • Applicant will submit a copy of the applicant’s physical therapy education diploma(s), degree(s), or certificate(s) that has been notarized as a “True copy of the original”.

  • The applicant will contact ETS and ask to have the TOEFL scores sent to FCCPT. The FCCPT institution code is 8164.

  • If applicant has taken the NPTE, contact the FSBPT for a score transfer.

  • Applicant may log in to check progress on file online, at any time.

  • Applicant must maintain accurate e-mail and mailing address.

  • The applicant will submit a notarized release of information if they wish a third party to contact the FCCPT in their place.
  • Applicants will NOT send original diplomas or certificates.
    • All documents sent to the FCCPT become the property of the FCCPT and will not be returned.
  • Applicant will NOT send any copies of transcripts, mark sheets, syllabi, or license verifications to FCCPT.

  • Applicants will NOT collect transcripts or verifications in sealed envelopes and forward these to the FCCPT.

  • Any documents that are not required will not be maintained in the FCCPT files.

What the applicant’s academic institution will do

Actions the academic institution should NOT take

  • The academic institution will fill out the academic information form and submit it with the full transcript and if required the official course descriptions and/or syllabi to FCCPT.

  • Documents should be provided in English if available.

  • Original language documents must be provided if translations are not available.
  • The academic institution should NOT send any documents to the applicant to forward on to FCCPT.

  • The academic institution should NOT forward documents to a translator to forward on to the FCCPT.

What the licensing authority will do

  • The licensing authority will fill out the license verification form and submit it to FCCPT.
  • The licensing authority should NOT send any documents to the applicant to forward on to FCCPT.

Actions the examining institution take

Actions the examining institution should NOT take

  • ETS will forward the applicant’s TOEFL, TWE, TSE, IBT scores to FCCPT.

  • FSBPT will forward NPTE scores directly to the FCCPT. (This is only required if the applicant already took the NPTE.)
  • The examining institutions should NOT send any documents to the applicant to forward on to FCCPT.

What FCCPT will do

  • FCCPT will supply the applicant with a file number once an online payment and application have been submitted.

  • FCCPT will send original language documents to the applicant once they are verified with instructions for the applicant to arrange for translations.

  • FCCPT will collect and verify ALL documents as they arrive.

  • FCCPT will complete the Type 1 review after all documents have arrived within 8 weeks time.

  • FCCPT will send the final report to the applicant and the jurisdiction requested electronically.

  • FCCPT will mail Type 1 certificates when applicant meets all the requirements.
  • For the Type I service, FCCPT will NOT request the academic, licensing, or examining institutions on behalf of the applicant.
    • FCCPT may contact academic institutions, licensing / registration agencies if further verification is needed.
  • FCCPT does NOT issue a license.
    • The jurisdiction board does this.
  • FCCPT does NOT issue the visa.
    • USCIS does this.
  • The FCCPT will NOT give out specific information to any third party without a notarized release of information from the applicant.
