Type I Certificate

This service is designed for the individual who has never been licensed to practice physical therapy in the U.S. and who needs a Health Care Worker Certificate to obtain USCIS clearance

Type II Certificate

Applications suspended as of September 01, 2009.

Educational Credentials Review

Used primarily for licensure to practice physical therapy in the U.S.

New York Verification

Used if you are a foreign-educated physical therapist or physical therapist assistant seeking a license to practice in New York state

PTA-EER

Used primarily for Centers for Medicare and Medicaid Services (CMS) certification and for a license to practice as a physical therapist assistant

Here is a brief description of each of the primary services that FCCPT currently offers. On the menu to the left, select the service that you want to apply for to learn more about that service.

Comprehensive Credential Evaluation Review (Type I Certificate)
This service is designed for the individual who needs a Health Care Worker Certificate to obtain USCIS clearance, which will permit employment as a physical therapist in the U.S. This service includes an educational credentials review, which can be used to obtain a license to practice physical therapy in the U.S, or for CMS certification as a Medicare/Medicaid provider.

Visa Credentials Review Service (Type II Certificate)
This service was designed for foreign-educated physical therapists that do not hold a Type I certificate, but do hold a current physical therapy license to practice in the U.S., and need a Health Care Worker Certificate to maintain visa status. Applications for the Type II process were suspended September 1, 2009.

Educational Credentials Review Service
Educational Credentials Review Service The educational credentials review is used for application for licensure to practice physical therapy in the U.S. It determines whether your education is substantially equivalent to an accredited physical therapy degree program in the U.S. This may also be used for admissions requirements into some U.S. educational programs or for CMS certification as a Medicare/Medicaid provider.

New York Verification Process
New York Verification Process The New York State Education Department (NYSED) requires applicants for licensure as physical therapists and physical therapist assistants to obtain independent verification of the authenticity of their credentials from a NYSED-approved Credential Verification Organization (CVO). FCCPT is an approved CVO by NYSED.

Physical Therapist Assistant Educational Equivalency Review (PTA-EER)
Physical Therapist Assistant Educational Equivalency Review (PTA-EER) The Physical Therapist Assistant Educational Equivalency Review is used for Centers for Medicare and Medicaid Services (CMS) certification and for a licensure applications for physical therapist assistant, where permitted by the jurisdiction.

Planned Learning and Assistance Network (P-L-A-N)
Planned Learning and Assistance Network is a related service to assist individuals identify possible options to supplement education. This is offered to individuals who have used FCCPT as well as those who used outside agencies for their reviews.
