There are a few related services that may be applicable to you during the process of completing an online application.
Duplicate Reports Apply Now
The Comprehensive Credential Evaluation Review (Type I Certificate), the Educational Credentials Review Service, and the Physical Therapist Assistant Educational Equivalency Review (PTA-EER) services each come with one free included state evaluation report that can be sent to a state licensure board, individual (such as a recruiter), or academic institution.
Additional evaluation reports, called duplicate reports, can be purchased during the online application process or after the evaluation service has been completed. The applicant will not receive a copy of the duplicate report. The fee for each duplicate report is U.S. $90 per report.
If an applicant is requesting a report to be sent to a state (Alabama) that requires a conversion to a specific state evaluation tool, AFTER the initial evaluation report has been finished, then a re-evaluation is required. The re-evaluation fee is U.S. $350. The state specific surcharges are included in this re-evaluation fee. To learn more about re-evaluations, go to “Returning Applicants” / “Re-Evaluation”.
Request for Copies
Apply Now
Applicants can also order a request for copies of documents (TOEFL scores, transcripts, syllabi, non-U.S. license verifications) in their file to be certified and sent to a third party or to themselves. Applicants should note that once documents have been sent to the FCCPT, these are the property of the FCCPT. Copies sent to applicants and jurisdictions (except New Jersey) are sent via e-mail unless the applicant specifically requests otherwise. The fee for this service is U.S. $25.00. The FCCPT final report will not be copied and certified, you would need to request a duplicate report.
