What the applicant will do

What the applicant will NOT do

  • Applicant will apply and pay online for the Educational Credentials Review Service.

  • Applicant will log into account and “view progress” to determine what checklist items are to be requested and sent to the FCCPT.

  • Applicant will keep e-mail address up-to-date at all times.

  • Applicant will send academic and licensing request forms to the appropriate Institutions.
  • Applicant will submit a notarized attestation page for the Educational Credentials Review Service with photo to FCCPT.
  • Applicant will send a notarized copy of the PT diploma to FCCPT.

  • The applicant will submit a notarized release of information if they wish a third party to contact the FCCPT in their place.
  • Applicants will check with the jurisdiction for any updates in requirements prior to application.

  • Applicants will pay the additional $50 surcharge for a state specific conversion.
  • Applicants will NOT send original diplomas or certificates.
    • All documents sent to the FCCPT become the property of the FCCPT and will not be returned.
  • Applicant will NOT send any copies of transcripts, mark sheets, syllabi, or license verifications to FCCPT.

  • Applicants will NOT collect transcripts or verifications in sealed envelopes and forward these to the FCCPT.

  • Any documents that are not required will not be maintained in the FCCPT files.

what the Academic institution will do

Actions the academic institution does NOT take

  • The academic institution will fill out the academic information release form and submit it with the full transcript and official course descriptions and/or syllabi to FCCPT.
  • The academic institution will not send any documents to the applicant to forward on to FCCPT.

What FCCPT will do

Actions FCCPT does NOT take

  • FCCPT will supply the Applicant with a file number once an online payment and application have been completed.

  • FCCPT will complete the review and issue a report to the applicant via e-mail along with a report to one state as requested.
    • Duplicate reports can be requested as related services.
  • FCCPT will include in the final report the practice requirements in country of education, the applicant is not required to provide evidence of licensure for this service.

  • The FCCPT will complete the review after all documents have been verified within 8 weeks time.
  • For the Educational Credentials Review service, FCCPT will not be contacting any academic, licensing, or examining institutions on behalf of the applicant. However, FCCPT may contact academic institutions, licensing / registration agencies if further verification is needed.

  • FCCPT does NOT issue a license.
    • The jurisdiction board does this.
  • The FCCPT will NOT give out specific information to any third party without a notarized release of information from the applicant.
