Re-evaluations
Applicants may request a re-evaluation after an evaluation report has been issued.
There are 5 different types of re-evaluations that apply to the PT or PTA applicant.
Applicants are encouraged to read carefully each of these types of re-evaluations to be sure they order the correct service online.

Re-evaluation based upon additional information Apply Now Download Re-evaluation Statement Form
If it is determined that the applicant did not submit all of the necessary information prior to the educational credential review beginning, or that the information originally sent was not complete or incorrect, the applicant can arrange to have additional information sent to FCCPT by the appropriate issuing institution and the applicant can apply for a re-evaluation.
Additional information may consist of curriculum/course descriptions for courses on transcripts, evidence of missing courses from transcripts, clinical education certificates, licensure documents, etc. All additional documentation must come directly from the issuing institution and not from the applicant.
FCCPT will only accept pre-existing documents from the school. We will not accept letters indicating content has been taught and we will not accept documents that are prepared specifically for FCCPT’s purposes. If a more detailed syllabus is being submitted for re-evaluation, it must be the syllabus in existence at the time of the applicant’s attendance. It must include the entire PT curriculum, and it also must include a full bibliography of text books used in each class. If this information was not provided at the time of the initial evaluation, it must be accompanied by a letter of explanation as to why this information was not sent with the original set of documents. This letter must be signed and sealed by the appropriate person at the university or college. If the new documents fail to meet these criteria, they will not be used in the re-evaluation process.

If you have taken additional coursework since your initial evaluation and would like to have it evaluated, then you must request a re-evaluation service.
FCCPT must receive all official transcripts and syllabus (if physical therapy coursework) directly from the issuing institution in order for it to be considered for re-evaluation.
The syllabus for any physical therapy course is required. FCCPT will only accept pre-existing documents from the school. We will not accept letters indicating content has been taught and we will do not accept documents that are prepared specifically for FCCPT’s purposes.

If an applicant believes that an error was made in the educational credentials evaluation report, then the applicant must apply for a re-evaluation service. If you wish FCCPT to compare an evaluation completed previously with your evaluation, you must request a re-evaluation and provide the name and file number of the applicant you would like your evaluation compared to. FCCPT will only compare evaluations for applicants with the same university, college and graduation year.
If we find that FCCPT made an error in your initial evaluation, then a full refund of the re-evaluation fees will be sent to the applicant.

Re-evaluation based upon a conversion to a state-specific evaluation tool Apply Now Download Re-evaluation Statement Form
If your evaluation is on a coursework evaluation tool that the state you are seeking licensure in does not accept, you will need to request this type of re-evaluation to update your evaluation to the correct version of the coursework evaluation tool.

If you have participated in the PLAN-FULL service and have taken all the pre-approved coursework you need to apply for the PLAN re-evaluation service to have the coursework added to your evaluation.
