Important: Please read the information below before applying for a Re-evaluation.

Re-evaluation is for applicants who want to request a reassessment of educational credentials after an evaluation report has been issued.

You must apply for an Included Report within 15 days of application. You can notify FCCPT of a recipient for the Included Report, at no additional cost by submitting either the Re-Evaluation Statement or the Included Report Request Form, for up to 15 days following the receipt of the application.      

  • If you request a report recipient AFTER 15 days from the receipt of the application, you will have to apply for a Duplicate Report and pay the associated fee. 
  • If you did not select a recipient for the Included Report, FCCPT will complete your evaluation on the most current Coursework Evaluation Tool (CWT6). You will need to apply for a CWT Version Update if you choose later to have an evaluation sent to a state that accepts the tool based on your year of graduation.

 

Who should apply for a Re-evaluation?

If your previous evaluation was found to be ‘not equivalent’ you may apply for a Re-evaluation for any of the following reasons:

Re-evaluation based upon additional coursework

Re-evaluation based upon additional coursework

If you have completed additional coursework since the initial evaluation and would like to have it added to your evaluation report, then you can apply for a Re-evaluation.

Important:

  • FCCPT must receive all official transcripts directly from the issuing institution in order for the coursework to be considered for Re-evaluation.
  • A syllabus is required for physical therapy coursework to be included in the Re-evaluation.

If you have completed the PLAN-Full service and have taken all of the pre-approved coursework, you may apply for the PLAN Re-evaluation to have the coursework added to your evaluation report.

Re-evaluation based upon additional information

Re-evaluation based upon additional information

If FCCPT determines that either:

1) All of the necessary information was not submitted by an issuing institution prior to the start of the review process, or

2) The information originally sent was incomplete or incorrect, then you may request to have additional information sent to FCCPT from the appropriate issuing institution and apply for a Re-evaluation.

  • Additional information can include curriculum/course descriptions for courses on transcripts, evidence of missing courses from transcripts, clinical education certificates that include the number of hours in the internship, licensure documents, etc.

Important:

  • All additional documentation must come directly from the issuing institution and not from you (the applicant).
  • FCCPT will only accept pre-existing documents from the school. FCCPT will not accept letters indicating content has been taught or documents prepared specifically for FCCPT purposes.
  • If a more detailed syllabus is being submitted for Re-evaluation, it must meet the following criteria:
    • The syllabus is the syllabus that was in use at the time of the your attendance;
    • The syllabus must include the entire PT curriculum; and
    • The syllabus must include a full bibliography of textbooks used in each class.
  • If the information was not provided at the time of the initial evaluation, it must be accompanied by a letter of explanation as to why the information was not sent with the original set of documents. The letter must be signed and sealed by the appropriate person at the university or college.
  • If the new documents fail to meet the criteria, they will not be used in the Re-evaluation process.

If you believe that an error was made in the Educational Credentials Review, you may apply for a Re-evaluation.

Important:

  • If you want FCCPT to compare an evaluation completed previously for another applicant with your own evaluation, you need to apply for a Re-evaluation and provide the name and file number of the other applicant that you would like your evaluation compared to. FCCPT will only compare evaluations for applicants who attended the same university and/or college, during the same time period.
  • You must provide a statement listing the specific course and content that you think we did not include in your evaluation.
  • If we find that an error was made in your initial evaluation report, then a full refund of the Re-evaluation fee will be issued.
